Wealth Being: Members Application

I hereby apply for The Wealth Being Members Club. If accepted, I agree to be bound by the rules of the club. Please note that all sections must be completed in order for this application to be submitted to the committee. Any application missing information will be deemed incomplete and not considered. All information shall be confidential. The current membership dues and initiation fees are as follows: General Membership dues are 1,111 per annum,
plus a one-time initiation fee of ,500. Your credit card will not be charged unless your membership is approved. Both membership dues and initiation fees are subject to applicable taxes and services fees. This form will take around ten minutes to complete.
